Summary:

The Homemaker is responsible for aiding with Instrumental Activities (IADL’s) of Daily Living (examples: shopping, preparing meals, housekeeping, laundry, etc) to a designated group of patients in their homes as directed. The incumbent is responsible for para-professional aspects of care and patient safety. The Homemaker collaborates with the department leadership, family, and members of the interdisciplinary group. Performance reflects the mission and values of Home Care & Hospice.

Responsibilities:

  • Provides companionship, assistance with Instrumental Activities of Daily Living for patients as assigned and under the direction of Home Care & Hospice staff.

  • Recognizes symptoms and changes in the patient’s condition and provides feedback to the interdisciplinary team to assist in the evaluation of care.

  • Reports concerns about patients or family to the member of the interdisciplinary team who is coordinating homemaker services.

  • Provides service and produces work at or above expected levels of quality and consistent with agency defined qualitative standards.

  • Documents accurately, completely and in a timely manner.

  • Performs duties as assigned on the Homemaker Care Plan.

  • Participates in in-service and continuing education programs, staff meetings, and patient conferences as requested.

Other Information:

  • Age Minimum: 18 years old.

  • Must be reliable, punctual, neat, and organized.

  • Must be willing to perform tasks as requested, willing to learn job requirements, and able to follow directions.

  • Must have a valid driver’s license with reliable transportation and agency required auto liability insurance.

  • Must be able to drive within assigned region as directed.
