X Geriatric(s) 65 GENERAL DESCRIPTION :

The social worker assigned to the Mobile Crisis Team provides social work assessment, and crisis intervention and treatment to complex mentally ill patients who are at risk in a variety of outreach settings, including group homes, shelters, patients’ homes, and other sites across Monroe County. The social worker on the Mobile Crisis Team works closely with a Crisis Specialist. Through psychosocial assessment, crisis intervention, resource consultation, and brief treatment, psychosocial risk may be minimized and Emergency Department utilization decreased. The Social Worker participates in program policy planning, program development, program evaluation, and public relations activity.

SPECIFIC RESPONSIBILITIES :

Administrative (5%)

  1. Acts as a liaison between the Mobile Crisis Team, CPEP, Social Work Division, Department of Psychiatry, Strong Memorial Hospital, and community agencies.

  2. Attends monthly Social Work Division and Program Staff Meetings.

  3. Ensures that Social Work Division, Department, and Hospital policies and procedures are implemented with regard to professional practice, discharge planning and patient care.

  4. Participates in activities of the Social Work Division including, but not limited to timely completion of statistical reports/forms, time sheets and other Hospital or Division forms; Hospital and Division mandatory inservice training programs and UHS reviews.

  5. Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.

  6. Participates in available cultural competency events and training appropriate to job duties.

Clinical (95%)

  1. Provide psychosocial assessment to patients in outreach settings (i.e., group homes, shelters, patients’ homes) who are at exceptional psychosocial risk.

  2. Provides psychiatric crisis intervention and treatment in the community to help stabilize patients.

  3. Provides clinical leadership to Mobile Crisis Team in the field. Exercises considerable independent judgment with respect to assessment and treatment of psychiatric patients in outreach settings.

  4. Responsible for assessing safety of scene prior to deployment of Mobile Crisis Team and accessing need for law enforcement services to ensure safety of Mobile Crisis Team in the field.

  5. Involves family and other mental health treatment providers as necessary in interventions.

  6. Promotes and ensures continued staff education around multicultural factors which affect care and treatment of patients.

  7. Ensures that proper treatment and referral protocols are followed with respect to child abuse, domestic violence, consultation with specialty clinics, and referral both within and outside of SMH.

  8. Documents social work treatment in patient's chart in compliance with Division, Department and Hospital policy.

  9. Participates in on-call activities of the Mobile Crisis Team, including after hours and daily on-call.

  10. Provides back up social work services as requested to other unit/programs during the absence of certain co-workers.

  11. Provides orientation and training to new staff in the Therapist or Specialist positions of the Mobile Crisis Team as needed.

Teaching (As Approved – may replace up to 2.5% of Clinical)

Based on experience and as determined by the social work supervisor may supervise social work graduate students, psychiatric residents, medical students, and family medicine residents assigned to the Psychiatric Emergency Program. Supervision relates to psychosocial assessment and crisis intervention with patients and family

Research (As approved – may replace up to 2.5% of Clinical)

The social worker may participate in social work and or interdisciplinary research endeavors as determined by the social work supervisor, in consultation with the Social Work Clinical Manager in Psychiatry.

PROFESSIONAL SUPERVISION, ACCOUNTABILITY, AND EVALUATION :

Supervision/Consultation

The social worker's professional performance is supervised by a Social Work Division supervisor who is licensed by the State of New York (LCSW).

Accountability

  1. The social worker is accountable administratively and clinically to his/her social work supervisor (and through him/her to the Social Work Division Clinical Manager in Psychiatry) for provision of social work services, and adherence to Social Work policies, procedures, and standards of professional conduct. The social worker's job performance must conform to the expectations set forth in the position description.

  2. The social worker is accountable to the Coordinator of the Mobile Crisis Team for providing professional social work services in accordance with the policies and standards established by the Program and the Department of Psychiatry.

Evaluation

Social work evaluations are conducted annually by the social worker's supervisor, with review/comments from the MD working with the MCT, and the Program Director of the CPEP. Salary and promotional recommendations are made by the social worker's supervisor in consultation with the appropriate Social Work Division personnel. Salary increases and professional recommendations are subject to guidelines established by the Social Work Division, Strong Memorial Hospital and the University of Rochester.

POSITION QUALIFICATIONS :

The social worker who occupies this position must have a MSW from an accredited social work graduate program and must obtain New York State social work license within eighteen months of employment. Computer literacy preferred.

POSITION DESCRIPTION MAINTENANCE :

This position description is reviewed annually by the social worker and his/her supervisor. Position review provides opportunity for comment from the Clinical Director of the Program and for updating, as needed. Similarly, if a new social worker is hired for the position, a review and update of the position description should occur. Changes in the position description are recommended by the supervisor, in consultation with the Clinical Director of the Program. Any position description modifications are authorized by the Social Work Division Clinical Manager for Psychiatry.

Pay Range

Pay Range: $ 57,595 - $ 80,600 Annually

The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
