Two Care Coordinator Positions

Crisis Care Coordinator: Provide case management services to individuals in a mental health crisis. Assist in securing shelter, food, clothing, medicine, or other mental health services. Assist master level therapist in deescalating tense situations and monitoring consumers until situation is calmed. Meet with consumers being discharged from mental health hospital to address immediate needs and follow-up services. Provides transportation for consumers utilizing facility vehicle as needed.

Care Coordinator-Jail: Provide case management services to individuals in the county jail that have a mental health issue. Assist in securing housing, benefits, continuing mental health services and other resources as the individuals are released from the jail system. Work closely with master level therapist and other community service providers in an effort to decrease the potential of returning to jail.
