Responsibilities:

• Provide counseling and crisis intervention to patients and clients facing social or emotional challenges.

• Work across various settings, including hospitals, clinics, and community-based programs.

• Advocate for patients’ rights and access to resources.

• Conduct psychosocial assessments and provide ongoing support to patients and their families.

• Address issues related to mental health, substance abuse, trauma, and other social determinants of health.

• Maintain accurate and timely documentation of interactions and interventions.

  Qualifications:

• Master’s degree in Social Work (MSW) from an accredited program.

• Current state licensure as a Licensed Clinical Social Worker (LCSW).

• Experience working with diverse populations and across different age groups.
